LVN Intake Coordinator

Lubbock, TX

Job Summary: Managing patient admissions, coordinating schedules, and ensuring smooth patient transitions into care services. This position requires a licensed vocational nurse (LVN) with strong organizational skills, attention to detail, and the ability to communicate effectively with patients, families, and healthcare providers.

  

Essential Functions:

  • Conduct initial patient assessments to gather necessary medical information.
  • Coordinate with physicians and other healthcare professionals to determine appropriate care plans.
  • Schedule patient appointments, treatments, and consultations according to physician recommendations.
  • Ensure that patient schedules are optimized for efficiency and patient satisfaction.
  • Communicate appointment details and changes to patients and their families.
  • Maintain accurate and up-to-date patient records and documentation.
  • Ensure compliance with healthcare regulations and organizational policies.
  • Provide patients and their families with information about treatment procedures, medications, and home care instructions.
  • Address patient concerns and inquiries in a timely and professional manner.
  • Collaborate with interdisciplinary healthcare teams to ensure coordinated and comprehensive patient care.
  • Communicate effectively with physicians, nurses, and other healthcare professionals to facilitate seamless patient transitions.
  • Monitor and evaluate patient outcomes and satisfaction.
  • Identify areas for improvement in patient care processes and recommend solutions.

 

Additional Responsibilities:

  • Performs other duties as assigned or requested.
  • Conforms to all applicable Agency policies and procedures.
  • Participates actively in continuing education and in-services.
  • Maintains confidentiality of patient information and business trade practices
  • Assumes accountability for reporting incidents and complaints according to Agency policy.

  • Ability to develop strong working relationships and possess team organization and community resources coordination skills.
  • Sound clinical knowledge base.
  • Strong customer service skills, including proactive identification and resolution of customer issues using sound judgment and decision-making skills within parameters established by the Manager.
  • Strong prioritization and time management skills.  Must be detail-oriented.
  • Ability to work with minimal supervision and utilize clear, concise, and positive communication skills.

Minimum Position Qualifications:

Education: Graduation from an accredited school of vocational nursing.

License/Certification: Current LVN license in the state of practice.

Experience: Previous experience in a healthcare setting, preferably in intake coordination or scheduling roles.

 

Environmental Conditions:

Works under a variety of conditions in facilities and offices; ability to work flexible schedule, ability to travel locally; some exposure to unpleasant weather.  Moderate noise level; tasks may involve exposure to bloodborne pathogens; moderate stress and emotional demands.

 

Physical Requirements: Sitting is required. Requires ability to always handle stressful situations in a calm and courteous manner. Requires working under some stressful conditions to meet deadlines and agency needs. Ability to travel.
