Patient Care Manager (RN) Hospice

We understand that part of being a great Patient Care Manager (RN) is spending the time to genuinely understand the patient’s story. Knowing where the patient is coming from helps us determine what they need to feel comfortable, safe, and healthy in their own home.
General Purpose: To supervise all clinical operations within an office, including supervision of employees, to ensure compliance with all quality standards, laws, and regulations as related to patient/client care.

Essential Functions:
- Supervises develops, motivates, and evaluates clinicians and caregivers.
- Maintains a work environment conducive to achieving professional goals among the employees (i.e. training, educational resources, etc.).
- Provides orientation and training to employees.
- Supervises assigned clinical operations and ensures compliance with federal, state, local laws, and Interim HealthCare policies, procedures, and standards (i.e. chart audits, physician orders managements, etc.).
- Participates in clinical and operational QAPI activities and quality benchmarking reporting.
- Is responsible for providing ongoing in-service education from home health aides/CNA’s to meet annual requirements.
- Coordinates and develops in-service education for professionals as needs are identified.
- Ensures all established policies, procedures, and standards of Interim HealthCare are consistently followed by employees of an assigned business operation.
- Creates and/or implements employee recognition, reward, and incentive programs (in conjunction with the Director of Healthcare Services)
- Promotes, coordinates, educates, and contacts existing and prospective clients to review Interim HealthCare’s services, assess needs, and promote additional services.
- Ensures timely admissions within Interim guidelines of same-day admissions.
- Establishes appropriate standards of care and Interim HealthCare performance standards (in conjunction with the Director of Healthcare Services) to ensure patient/client and field employee expectations are met or exceeded.
- Assists with implementation of new company-sponsored systems.
- Will make clinical visits as necessary to meet patient needs.
- Shares in On-call rotation for Clinical Visits to include evenings, weekends, and holidays.
- May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities.
Minimum Education & Experience Requirements:
- 3 years of current Hospice nursing experience
- 1 year experience as a nursing supervisor preferred
- Unencumbered, Texas RN licensure required

- Meets applicable health requirements to provide patient care
- Ability to effectively communicate with clinical, non-clinical staff, patients, and family/caregivers
- Demonstrated ability to work under pressure with multiple tasks, changing priorities, and short deadlines
- Demonstrated excellent supervisory skills
- Demonstrated excellent organizational skills and attention to detail
- Computer proficiency to include current software programs used by the Company
- Ability to engage in local and out-of-town travel
- Valid driver’s license, if applicable
- Proof of Covid vaccine, medical exemption or religious exemption.
